Intake Form

Click the INTAKE FORM that applies to you.

When the form is complete click SUBMIT and I'll get to work!

NOTE: If you own a business or preformed any gig / side work, you will need to fill out a "PERSONAL TAX INTAKE FORM", and a "SCHEDULE C".

Once Intake form(s) are submitted I will contact you with any questions and to set up a remote meeting.
